Parking

Please be sure to park in a Rainier Properties Visitor Parking space or park on the street. Do not park in a permit parking spots! Towing is being strictly enforced. Street parking is pay to park. There are a few visitor spaces in front and more additional visitor parking spaces located behind the Nob Hill business building. To access the spots behind the building, drive to the cul-de-sac at the end of the street. You will see a narrow parking entrance to the left.

The business building you need to enter is the one directly in front of the blue USPS collection box. Take those stairs or elevator up to the last floor. Once you arrive at our suite 412, please wait in the seating area until I come to greet you as I could be wrapping up with another patient. Enjoy any reading available, grab some tea/water, or use the restroom while you wait.

Forms & Consents

All forms and consents must be signed before treatment begins. The booking, electronic health records, payment system, and email communication is HIPAA-certified. I understand that sometimes there are technical issues or you may not be able to fill out the forms well in advance of your appointment. If this is the case, please contact me. You may also arrive 20 minutes before your appointment for time to fill out the forms and agreements electronically (via ipad) in the office. However, please note that there is no receptionist at this location - it’s just me! and I could be with another patient for these 20 minutes before your appointment. I’ll do my best to greet you and get you started on forms if this is the case.

Insurance

Insurance is not taken at this time. Please request a SuperBill if one is needed for insurance reimbursement. You can use your HSA/FSA funds for these services as eligible reimbursements.

Payment

A credit card must be on file in order to book online for contactless payments. You can change the credit card on file at anytime. Cash is not accepted at this time. Full payment is due at the end of the service.

Gratuity

Pricing of the services listed was determined carefully and intentionally to avoid the necessity of tipping. While tips or gifts are appreciated, they are not to be expected.

Gift Cards

Gift Cards can be purchased directly through the Jane portal by clicking here. Non-patients are welcome to purchase gift cards and gift them to current patients or new patients. These gift cards will expire within 3 months of the purchase date.

Cancellation Policy

Your appointment time is reserved just for you. A late cancellation or missed visit leaves a hole in the schedule that could have been filled by another patient. As such, I require 24 hours notice for any cancellations or changes to your appointment. Appointments may be cancelled without penalty at least 24 hours in advance. Later notice will result in a charge to the credit card on file equal to 50% of the regular cost of the service. If the appointment is rescheduled for another time that same day and we can accommodate it, the fee is waived!

"No shows” will be charged the full service fee, as this directly impacts and prohibits us from servicing others. I understand that emergencies, illnesses and other issues do suddenly come up. Please call, text, or email me as soon as you know your appointment needs to be adjusted. Exceptions may apply.
